When you have to get up to 250 patients through anti-coagulation testing within 3 hours you need a testing system that is accurate, reliable, simple to use and cost effective, that is why the Haematology Department at Russells Halls Hospital chose the thrombi stat MC1 and MC4 as their INR testing systems.

“During a busy clinic 3 members of staff can test up to 5 patients at any one time, that is why the testing system and the reagents have to be easy to use and give us the results we are expecting,” explains Paul Leyshon, the Assistant Head of Blood Monitoring Services at the hospital. “During each clinic we use a thrombi stat MC1 and MC4 which we are able to calibrate with our main laboratory testing systems, to give us the confidence to know that the results we are seeing are accurate and reliable.”

The thrombi stat system uses the Manchester Capillary Reagent Kit, which is quick and easy to use and gives test results in less than two minutes and when used in combination costs as little as 50p per test to undertake, making it extremely cost effective for a clinic with up to 7000 patients to test on a regular basis.

“Numerous INR testing and reagent companies come and talk to us about their products but we find their technology in terms of accuracy and reliability is no better than the Manchester Capillary Reagent testing system and the costs associated with these other reagents outweighs any incremental benefits.” explains Paul Leyshon.

Using in Primary Care

John Low Ltd, a community pharmacy based in Country Durham, has run an anti-coagulation clinic on behalf of the health authority and subsequent PCT for over 8 years. They now have a throughput of approximately 45 patients on a weekly basis and receive payment for this service under the National Enhanced Service scheme from NHS County Durham and Darlington.

When they first set up the clinic they decided to use Hart Biologicals’ thrombi stat INR testing system that is compatible with the Manchester Capillary Reagent Kit, one of the leading wet chemistry reagent used in Warfarin management in the UK.

“We have a thrombi stat MCIWB INR testing system, which is a single channel system and ideal for a clinic such as ours,” explains Dawn Cruickshank, the Superintendent Pharmacist at the pharmacy. “The system is extremely easy to use and you are able to set everything up before the clinic starts which allows the clinic to run smoothly and efficiently.”

“Hart Biologicals have also provided us with many years of excellent customer service support, so much so that when the local NHS tender for anticoagulation services for the area was issued, I did recommend thrombi stat to several of my pharmacy colleagues, as it saves us an annual cost of £50 per patient.

Based on having 189 patients who used the clinic, John Low saves £9,450 by using the thrombi stat INR testing system, in comparison to using a CoaguChek® system. This is because handheld devices that use dry chemistry add a significant cost to anticoagulation monitoring as each test strip costs around £3, yet there is no difference in the level of accuracy.

GP Clinic

MC1WB in Primary Care

Like many primary care practices, The Robin Hood Practice in Sutton, Surrey, works with four surrounding practices to provide a community-based anti-coagulation clinic, as many of their patients prefer visiting a GP practice rather than going to their local hospital on a regular basis.

“At the moment we have approximately 310 patients coming to the clinic and that number is growing all the time,” explains Dr Ravi Seyan, who is responsible for the clinic which is run by his practice nurses. “Having a community-based anti-coagulation clinic better serves our patient population, as some need to have their blood checked every fortnight in the early stages of being initiated on Warfarin, so constant trips to the local hospital, can be quite an arduous task for patients especially if they are elderly.”

The team had previously used the CoaguChek XS® system in the clinic setting but decided to change over to Hart Biological’s thrombi stat MC1WB testing system due to its cost effectiveness. Dr Seyan identified that they would save over £10,000 by using the thrombi stat MC1WB testing system and this figure would increase as the numbers of patients going through their clinic simultaneously increased. The thrombi stat MC1WB system provided them with the efficacy and accuracy required in coagulation monitoring but was significantly cheaper because the consumables and reagent cost only 50p per test. CoaguCheck XS® is still used for all domiciliary visits.

The team were also fortunate that one of their newly appointed nurses already had experience in using the thrombi stat MC1WB testing system, who confirmed to them the ease of use of the system, which gave them the reassurance to purchase a thrombi stat MC1WB. Hart Biologicals also provided the team with a half-day training session, so all the staff were confident in using the system and collecting the blood samples from patients. “We have been delighted with the support we have received from Hart Biologicals,” explains Dr Seyan. “They have worked with us through the whole implementation process and now we are running a very successful cost-efficient, community-based anti-coagulation clinic.”

How to set up thrombi stat MC1

Step 1

  • Allow reagent vials and reconstitution fluid to come to room temperature.
  • Leave on bench overnight.
  • Switch on thrombi stat MC1.
  • Allow to warm up for 15 minutes.

Step 2

  • While the thrombi stat MC1 is warming up reconstitute the required vials of reagent.
  • Allow to stand for a minimum of 15 minutes.
  • Mix thoroughly before use. Do not allow to settle.

Step 3

  • While the thrombi stat MC1 is warming up reconstitute the QC plasma by adding 0.5ml of purified water to each vial (4 shots from the reagent pipette).
  • Allow to stand for 10 minutes.
  • Swirl to mix.

Step 4

  • While the thrombi stat MC1 is warming up place the required cuvettes in the rack and dispense a ball into each cuvette

Step 5

  • Dispense 125 µl of well mixed reagent into each cuvette.

Step 6

  • Insert one cuvette into the measurement cell of the thrombi stat MC1.
  • Allow to warm for 60 seconds.

Step 7

  • Test the QC plasma.
  • If the QC plasma is in range you are ready to test patients.
  • If results are out of range re-test and check for mistakes in preparation.

How to take a reading with thrombi stat MC1

Step 1

Collection of a good quality capillary sample is critical for the accurate measurement of INR.

A warm hand and good flow to the puncture site are essential to collect a good capillary sample.

To warm the hand you can,-

  • Wash the hand with warm water.
  • Apply a warm compress for several minutes.
  • Gently massage the finger from base to tip several times to bring the blood to the finger tip.

Step 2

  • While the thrombi stat MC1 is warming up reconstitute the required vials of reagent.
  • Allow to stand for a minimum of 15 minutes.
  • Mix thoroughly before use. Do not allow to settle.

Step 3

  • While the thrombi stat MC1 is warming up reconstitute the QC plasma by adding 0.5ml of purified water to each vial (4 shots from the reagent pipette).
  • Allow to stand for 10 minutes.
  • Swirl to mix.
